PLAYER REGISTRATION FORM: 
This form is used for players to register with a team for a season.  The player needs to fill out the form and submit it to their coaches/managers, who are then to submit the collected forms to the club registrars.

TRAVEL PERMITS: 
This form is used when a team wishes to apply to a tournament or festival outside of the city, and can be completed online through the CTMS website.
WHAT TO DO: Follow the link below which will bring you to the CTMS website.  You will need to create a login in order to complete the online application.  Once you have completed the online application you will need to notify your club approval representative.  They will approve the form online, and then we at the district will approve the form.  You should receive notifications of when your applications have been approved.  Once approved you will be able to go back onto the site and print the form directly from there, no stamps or signatures needed.

UPDATE: For tournaments to the USA please note that there are now mandatory fields indicating an upload for proof of travel insurance and competition sanction approval.  The sanction forms are to be obtained from the tournament that you are applying to and can usually be found on the tournament website - if not you will need to email the tournament coordinator to obtain the form.  Proof of travel insurance can be provided by any company or you can use the link below to apply for travel insurance from HUB

FEE: The charge to have travel permits approved is $20.00 per permit. This can be paid in the office by CASH OR CHEQUE ONLY.  You may also mail in a cheque to cover the cost but we ask that you please include your club, team, and the name of the tournament with the cheque for our records.

TEMPORARY PERMITS: 
This form is used when a team wishes to temporarily register a player who is already registered with another club for a league/exhibition game. 
WHAT TO DO: Print out the form below, fill in the information and bring the form into the office along with a player book for the player you are wishing to temporarily register.

SHORT TERM REGISTRATION PERMIT: 
This form is used when a team wishes to use a player who is NOT registered with another team for the purpose of tournament, league or exhibition game play. 
WHAT TO DO: Follow the link below to print out the form.  Fill in the required information as outlined in the form and bring the form into the office along with a player book for the individual, for approval.

UPDATE: Short Term Registration Permits are no longer approved right away in the office.  Ontario Soccer is approving all forms at this time, therefore once payment is received in the office a scan of the form will be sent to Ontario Soccer and you will receive the approved permit once they have reviewed it.  The turn-around time for this is usually 1-2 business days.

PLAYER TRANSFER/ DE-REGISTRATION: 
This form is used when a player wishes to switch from the club they are currently registered with to another club (For example: A player from Caboto S.C. to Polonia S.C.) or when a player needs to de-register from a team for the season.
WHAT TO DO: Print out the form below, fill in the information and bring the form into the office along with a player book for the player switching clubs or being removed from the roster.

UPDATE: Ontario Soccer is now approving all transfers/de-registrations.  The forms are still to be brought to the ECSA office where the applicable fee can be paid.  Once received we will scan and send to Ontario Soccer who will email back with approval.  Turn-around time is usually 1-2 business days.

FEE: The charge to have transfer permits approved in the office is $35.00 per permit. The charge for a de-registration is $5.00.  (CASH OR CHEQUE ONLY)


PLAYER MOVEMENT FORM: 
This form is used when two clubs wish to affiliate with one another.
WHAT TO DO: Print out the form using the link below, fill in the information and submit the form to the district office.  Once submitted the form is then forwarded on to OS.

INSURANCE REQUEST FORM
This form is used to obtain insurance from HUB international for rented spaces such as gyms or fields.
WHAT TO DO: Fill out the form and return to the district by email. The completed form will be sent to HUB International for approval.
